    How to Access Supported Living.

    If you are a parent, family member or carer looking for supported living or specialist care for someone you love, or are seeking support yourself, it can be difficult to know where to start.

    Finding the Right Support

    Independent Lifestyles provides specialist supported living and residential care services for adults with complex needs, including autism, learning disabilities, acquired brain injury, mental health needs and behaviours that may challenge.

    We support people to live as safely, confidently and independently as possible, with care and support tailored around their individual needs, wishes and goals.


    Who Our Services Are For

    Our services are designed for adults with specialist support needs, including people who may need help with:

    • daily living skills
    • personal care
    • communication
    • emotional wellbeing
    • medication and health appointments
    • behaviours that may challenge
    • accessing education, work, volunteering or activities
    • building confidence and independence
    • living safely in their own home or a supported setting

    We provide support across Sussex and Kent, including supported living and residential care services.

    Contact our friendly team by telephone, email or through our website enquiry form. We will take some initial details and discuss the person’s support needs, current circumstances and what they are looking for from a future service.

    Getting to know you

    If we believe we may be able to meet the person’s needs, we will arrange an assessment. This gives us the opportunity to spend time with the individual, their family and any professionals involved in their support.

    We want to understand what is important to the person, their goals, preferences, support needs and aspirations for the future.

    Exploring the options

    Where appropriate, we will arrange visits to a suitable service so the person can see the environment, meet staff and learn more about the support available. This helps everyone decide whether the service feels like the right fit.

    Developing a support proposal

    Following assessment, we will provide recommendations based on the person’s individual needs and outcomes. Where appropriate, we will work with local authorities, commissioners, family members and other professionals to develop a suitable support package and funding arrangements.

    Planning the transition

    Once support arrangements have been agreed, we will work closely with everyone involved to plan a smooth transition. Our team will provide guidance and support throughout the process, helping the person settle into their new environment and build positive relationships with those supporting them.

    Independent Lifestyles does not provide general elderly home care, domestic home help or short visits for older people who need support with household tasks only.

    Our services are specialist care and support services for adults with complex needs, learning disabilities, autism, acquired brain injury, mental health needs and related support needs.

    If you are looking for elderly care, home help or general domiciliary care, your local authority adult social care team may be able to advise you about suitable services in your area.

    You are looking for general elderly home care, domestic help, cleaning, companionship visits or short-term home help.

    A good first step:

    Contact your local authority adult social care team and ask about a care needs assessment or supported living options.

    How to Start the Process

    The first step is usually to speak with your local authority adult social care team.

    If your loved one has eligible care and support needs, the local authority can complete or update a care needs assessment. This assessment helps identify what support may be required and whether supported living or residential care may be appropriate.

    If your loved one already has a social worker, care manager, transition worker or commissioner involved, they may be able to support the referral process and discuss suitable options with you.


    What Information May Be Needed?

    To help us understand whether we may be able to provide suitable support, it is useful to have as much information as possible about the person’s needs, wishes and current circumstances.

    This may include:

    • the person’s age and current living situation
    • diagnosis or support needs
    • current care and support arrangements
    • level of support required, including whether daytime, sleep-in, waking night or 24-hour support may be needed
    • communication needs
    • mobility, health or medication needs
    • behaviours that may challenge, risks or safeguarding considerations
    • what the person enjoys and what is important to them
    • family involvement and wider support networks
    • whether funding or a local authority assessment is already in place

    Do not worry if you do not have all of this information when you first contact us. We can talk through the situation with you and explain what may be needed next.


    Supported Living and Housing

    In supported living, the care and housing arrangements are usually separate.

    Independent Lifestyles provides the care and support package. Where appropriate, we may work alongside specialist housing providers who manage the accommodation and tenancy arrangements.

    This means that housing, tenancy and benefits matters are dealt with separately from the care and support we provide.


    What Happens After an Enquiry?

    Once we receive an enquiry, we will usually ask for further information so we can understand the person’s needs and whether we may be able to offer a suitable service.

    This may involve:

    • an initial conversation with the family, social worker or commissioner
    • reviewing care and support information
    • considering whether there is a suitable service or placement available
    • assessing whether we can safely and appropriately meet the person’s needs
    • discussing next steps with the relevant professionals

    Where we are not the right service, we will always try to be clear and helpful about this.

    Working Together

    We understand that looking for the right support can feel overwhelming, especially when a loved one is preparing to move from home, school, college, hospital or another care setting.

    Our team will take time to listen, answer questions and explain the process as clearly as possible.

    Where a placement or support package may be suitable, we work closely with the person, their family where appropriate, local authorities, commissioners, health professionals and housing providers to help create a safe, stable and person-centred support arrangement.
